[99mTc]Tc-HYNIC-FAPI is a radiopharmaceutical imaging agent developed for SPECT/CT imaging of various metastatic cancers, including lung, pancreatic, breast, and colorectal cancers. It consists of a fibroblast activation protein inhibitor (FAPI) conjugated with the chelator 6-hydrazinylnicotinic acid (HYNIC) and radiolabeled with the gamma-emitting radioisotope technetium-99m. The agent specifically targets fibroblast activation protein (FAP), a cell-surface glycoprotein that is highly overexpressed on cancer-associated fibroblasts (CAFs) within the tumor stroma of most epithelial carcinomas. By binding to FAP, the tracer allows for the sensitive detection and localization of primary tumors and metastatic lesions using nuclear medicine imaging. Developed by the Tomsk National Research Medical Center of the Russian Academy of Sciences, it is currently undergoing Phase 1 clinical trials to evaluate its safety, dosimetry, and biodistribution in patients with advanced malignancies.
